Windows 10 In-place Upgrade reverts back to Windows 7

We've been using PDQ Deploy and Inventory to do a Windows 10 In-place upgrade and it's been going well. Here's what I followed if anyone is curious:

I have a workstation that goes through package for the upgrade successfully, shows Windows 10 in Inventory and then about 5 minutes later reverts back to Windows 7. I've tried to upgrade this workstation at least five times and it always reverts back.

Does anyone have an idea of what is causing this?

  • Hi There! I'm seeing the same issue. On reboot the computer will throw a "Can't install windows 10" error with these details: "The installation failed in the SAFE_OS phase with an error during APPLY_IMAGE operation. Any hints?
